In accordance with my request to establish service with GTC Telecom, I
understand and agree to the following Terms and Conditions:

My intraLATA, interLATA, and international long distance (and local toll,
if applicable) telecommunications services will be provided by GTC Telecom.
I have selected GTC Telecom as my primary long distance carrier (and local
toll, if applicable) and will be responsible for all charges due for such service.
GTC Telecom's long distance service includes in-state, state-to-state, and
international long distance call traffic. In-state long distance rates may
differ from state-to-state rates and international rates.
The rates charged for residential and business services are billed in one minute
increments. This includes 1+Dialing, inbound toll free dialing, travel cards
and pre-paid calling cards.
In addition to the charges specifically pertaining to GTC Telecom's service,
applicable federal, state and local taxes, fees and surcharges will apply and
will be billed as separate line items on my monthly bill. GTC Telecom's current
USF surcharge rate is 8.3%.
GTC Telecom reserves the right to examine my credit record and check my
references. The service application shall not in itself obligate GTC Telecom
to provide services or to continue to provide services if a future check of
my credit record is, in the opinion of GTC Telecom, contrary to the best
interest of GTC Telecom.
My service is provided and billed on a monthly basis, and the billing date is
dependent on my specific billing cycle. Billing is due upon receipt, and a
late fee of 1.5% monthly or the amount otherwise authorized by law, whichever
is lower, will be charged on any past due balances.
